    Does K-Wire Fixation of Hand Fractures Outside the Main Operating Room Increase Infection Rates?

    Background: Closed reduction and K-wire fixation (CRIF) of closed hand fractures in the main operating room (OR) is much more expensive than outside of the OR. However, there is a reluctance due to the perceived increase in infection rate. Our goal was to prospectively analyze the complications and infection rate of performing CRIF of close metacarpal and phalangeal fractures in these 2 settings.

    Methods: A fully powered multicenter prospective analysis of patients undergoing CRIF of the metacarpal or phalangeal fractures inside or outside the OR was performed in 19 cities. Demographic data, injury characteristics, surgery information, and postoperative infectious complications were recorded, including cellulitis, frank pus, and osteomyelitis.

    Results: The study involved 1042 patients with a total of 2265 K-wires. The OR group had a longer time to operation and longer procedure time, but a shorter time with the K-wires in place. There was a higher risk of pin site inflammation if done in the OR, however, the inf

    Summary of Activity: Dr. Edward (Ted) Tredget received his medical degree with distinction from the University of Alberta, Canada and completed his internship, general surgery and plastic surgery training from the University of Alberta.  He undertook a Post Doctoral Fellowship at the Massachusetts Institute of Technology, Massachusetts General Hospital and Harvard Medical School where he received his Masters of Science degree. Currently, Dr. Tredget is the Director of the Firefighters' Burn Treatment Unit  at the University of Alberta Hospital and the Burn Wound Healing Research Laboratory of the University of Alberta.  The focus of their research is basic science research is wound healing, hypertrophic scarring and tissue engineering after burn injury.  Their work has explored the cellular, proteomic and immunologic features of hypertrophic scarring using biobanked tissues, large and small animal models, and in vivoclinical trials in human volunteers and burn patients.
